Modified Soft Har Gow (Dim Sum)
Har gow (shrimp dumplings) filling minced to ≤4 mm particles and served without the wrapper — a culturally meaningful IDDSI Level 5 (Minced & Moist) option for Cantonese patients.
Natural IDDSI Level
Standard har gow is not dysphagia-safe — the rice flour wrapper is chewy (Level 6) and the whole shrimp filling is Level 7. The modified version extracts the shrimp filling, minces it to ≤4 mm, and discards or separates the wrapper. Served with a light oyster sauce glaze, it meets Level 5 criteria while preserving cultural familiarity.
Safe Preparation
Steam har gow dumplings until fully cooked. Open the wrapper carefully and remove the shrimp filling. Discard or separate the wrapper (which is chewy and not Level 5 compliant). Mince the shrimp filling to ≤4 mm using a knife or food processor. Add a small amount of warm oyster sauce or light soy sauce to coat. Serve immediately.
- ✓Ask kitchen staff to prepare the shrimp filling separately without the wrapper for care home production.
- ✓Commercial minced shrimp pastes can substitute if fresh har gow is unavailable.
- ✓The oyster sauce coating helps bind particles and adds familiar Cantonese flavour.
- ✓Test particle size before every service — shrimp pieces can vary in size.
Important Warnings
- The wrapper must be completely removed — the chewy rice flour skin is a choking hazard at Level 5. Do not allow patients to receive any wrapper pieces.
Frequently Asked Questions
Why is modified dim sum important for Cantonese dysphagia patients?
- Cultural food familiarity significantly impacts dietary compliance and quality of life for patients on modified-texture diets. Cantonese patients who have eaten dim sum weekly for decades may experience significant distress when culturally meaningful foods are entirely removed from their diet. Modified dim sum offerings — even if only the filling — help maintain cultural identity, improve meal enjoyment, and support better overall food intake in care settings.
